Community Outreach Program
Housing Assistance: Pathways Community Outreach Services was created to provide practical support to individuals and /or families who are homeless, at risk of becoming homeless, experiencing a housing crisis, or requiring assistance to maintain accommodations.
The support will be provided in a timely, flexible and person-centred approach and will focus on reducing and preventing homelessness. Services will be provided based on the philosophy of encouraging self-work and independence.
Connecting Agencies: Pathways Outreach Services was designed to create, and maintain relationships with existing outreach services within the City of Lethbridge and to ensure all organizations providing outreach services have up-to-date information on programs and services, initiatives related to homelessness and emergency assistance.
By creating a team concept between outreach agencies, opportunities such as training, reporting mechanisms and problem-solving techniques will provide a "seamless" system of service delivery within interagency developed protocols.
The Need: Pathways Outreach support services, in combination with other short and long-term solutions, have been identified as one of the keys to ensuring people are able to secure and maintain the right housing options for their situation.
